About Guidant Care Management

Guidant Care Management, formerly known as Always Best Case Management has been providing Geriatric Case Management services and Assisted Living Placement to California seniors since 2006. Our passion for helping people maintain a safe, independent and dignified lifestyle represents the strong foundation of Guidant Care Management. We are one of the first Care Coordination Agencies to contract with the California Assisted Living Waiver Program (ALWP) when it began as a pilot in 2006. We have successfully transitioned and placed thousands of Medi-Cal eligible seniors and persons with disabilities in Los Angeles, Sacramento, and San Joaquin Counties.
